Rectal Gonorrhea (aka Anal Gonorrhea, The Clap) is an infection of the Rectum by the bacteria Neisseria Gonorrhea.

It can also infected the anus and this is called Proctitis.

How do I know I have it?

Rectal Gonorrhea can cause itching of the anus, blood or mucus when you pass motion, soreness of the anus or a constant feeling like you need to pass motion.

Sometimes, Rectal Gonorrhea causes no symptoms at all.

How can I catch it?

Rectal Gonorrhea is caught when you have unprotected anal sex with a partner who is infected with Gonorrhea.

If I have Rectal Gonorrhea can I pass it to my partner?

Yes. If you have Rectal Gonorrhea, you can pass it to your partner by having unprotected anal sex.

If your partner develops symptoms of Gonorrhea (pain passing urine, discharge) after having anal sex with you, you should have yourself checked for Rectal Gonorrhea.

How do I check for Rectal Gonorrhea?

Com see us. We will take a swab from your anus and send it to the lab for a special culture. Please let your doctor know you are worried about Rectal or Anal Gonorrhea. It requires a special swab.

Please know that the symptoms of Rectal Gonorrhea can be very similar to other diseases such as Ulcerative Colitis or Crohn’s Disease. Please ask Our Doctors for more advice.

How is Rectal Gonorrhea Treated?

The commonest treatment for Rectal Gonorrhea is an injection of an anti-biotic called Ceftriaxone. If you are unable to take this injection you will be given pills instead. You might need to visit the doctor again to make sure the Rectal Gonorrhea is fully cured.

How can I protect myself from Rectal Gonorrhea?

Always use a condom when having anal sex. Although this reduces the risk of Rectal Gonorrhea, it is not 100% protective.

Are there other STDs associated with Rectal Gonorrhea?

Yes. If you have Rectal Gonorrhea you likely will have Gonorrhea in other areas such as the Urethra (urine tube) or Throat. You should ask your doctor to check for these too.

Rectal Gonorrhea is associated with HPV (Human Papilloma Virus) infection of the anus and the peri-anal region (area around the anus). This may cause you to develop warts. Certain strains of HPV can also lead to anal cancer. Please seek Our Doctor’s advice on testing for anal HPV and cancer.

Rectal Gonorrhea is also associated with other STDs such as HIV, Syphilis and Hepatitis B.

  2. I was recently diagnosed with chlamydia of the penis and I am straight, never had anal sex. I was given the single dose medication and currently three days later, I now have this itchy feeling in my anus and semi loose stool. Can I have gotten chlamydia of the anus somehow?

    • No, this will not be from rectal chlamydia. And if you did have rectal chlamydia, it would have been treated anyway with the antibiotics. Antibiotics can cause loose stools. Do not worry.

      • Doctor,

        I was diagnosed with Rectal Chlamydia and Gonnorhea and was given an injection and Azithromycin immediately. Once confirmed I was given 7 days of doxycycline also. This made all symptoms dissapear for around two weeks. Since I have started to have anal discomfort again which comes and goes. It will be present for a week and dissapear for a few days at a time. I suspect I have Proctitis however would this be caused by the chlamydia/Gonnorhea as I received treatment or would I need further treatment? I’m in abdominal pain also and it is really dragging on now this issue started in October 2017. Thanks for any advice.

        • It would be best to see a doctor for proper assessment. We cannot determine if you have proctitis from your symptoms alone, much less whether it is caused by chlamydia or gonorrhea at this point.

    I have been diagnosed of Gonorrhea of the rectum and throat and thankfully I have began treatment…. for the few days I have seen some very good signs of getting better.

    but I have some questions .
    Can the same treatment used for urethral gonorrhea be used to cure anal gonorrhea ?

    And do I have take my drugs for as long as I want?.


    • The treatment course is the same for rectal and urethral gonorrhea. What do you mean take your drugs “as long as you want”? Take the dose prescribed by your doctor.

    How much would it cost me for an anal abscess treatment? I have this small boil near my anus and after reading many articles and seing pictures i am pretty sure that it is an Anal abscess.

    Please advise.

    • Hi,
      It really depends on the size of the abscess (if it is an abscess) and what kind of treatment we provide. A standard consult will range from S$35-60 and subsequent treatment such as an incision/drainage or electrosurgery may cost between S$80-S$240. An abscess would also require a course of antibiotics and some tests such as wound swab/culture.

        Thanks for the prompt reply you mentioned (if it is an abscess) does that mean it maybe another time of illness? I have a small boil near my anus and its been sitting there for a year now.

        • If it has been there for a year it then it probably isn’t an abscess. Best if you come down to our clinic so we can take look.

  6. Hello, I’ve been experiencing a pain in my butt about once every 2 or 3 hours. It feels like a sharp pain and just pulses once and hurts then it is gone for a while. The only anal sex I’ve ever had was over a yea ago and I haven’t noticed anything until now. It feels like it is on the muscle you use when you push to go poo.

    This is my only symptom, does it sound like it could be an STI or maybe something else?

    • No. Rectal gonorrhea and throat gonorrhea will not spread to the testes. However, if you do have throat or rectal gonorrhea there is a good chance you also have urethral gonorrhea which may cause epididymitis. You also cannot transmit gonorrhea from your penis if you do not have a gonorrhea infection of the penis.

      • Hi I’m Morses and I’m experiencing an itchy of my anus a few months until now but I’m more worried because if I make poooo. It’s painful.and after I wipe a tissue I saw a blood and in the side of the hole in my anus there’s a scratch and a little blood most especially when my poooo is about to out and the muscle well stretch it leave a blood after.. what I am going to do with it?

    updating my.status regarding My HIV test. luckily a Month later alls well.My question is I have been having Loose stools for a while now. The stools are light coloure pale looking with occasional whithess ulcers or visible leakage or anal discharges..just loose stools e to 4 time a day. Urine gets a bit darker occasionally.
    will be doing my Std test soon.
    Would like to know from your experience with this.. In Gonnorhea Chylamadia of Herpes.. does Diarrhoe have differnt colored stools.
    have read stools are light with Gonnorhea and dark with Chylamadia.
    I did the Urine test earlier. but i guess i will do the swab test to have better results.

  10. Hi there. I am a straight man and for a few months I have been having discomfort while having bowel movements and rectal bleeding while having bowl movements. I have never had unprotected anal sex, and in fact never had anal sex or fingered or anything there. Maybe a couple weeks before the pain I did have unprotected vaginal sex. Could I have possibly contracted rectal gonorrhea this way? I do have a colonoscopy scheduled for next week to check for other possible reasons behind these issues.

    Hello, I am a straight man and I had sex with a female sex worker over a 5 months ago. I put my penis in her anus and the condom popped at the tip and I pulled out immediately. Since then I have had mild discharge on and off at the tip of my penis. I have taken 6 Chlamydia & Gonorrhea urine test and all negative. I also noticed my poop has had mucus around it. Is it possible to have rectal gonorrhea even though I never received anal sex and if I were to transmit to me via my penis?

    I had protected anal sex with a guy and I was receptive partner. After he removed his penis from my anus, I saw the condom was intact and there was no breakage or leak. I washroom, I found an opaque white liquid coming out. I am sure it was not semen. I am worried and wish to know what could it be? Is it mucous?

  13. I received anal penetration from a partner’s penis with no ejaculation (I didn’t let him stay in long enough because it hurt). I have abdominal cramping 3 weeks later along with a clear discharge that turned white and pussy. Stools are loose and sometimes laced with white, except when diarrhea is especially bad. I had an ste panel test and came up negative on everythjng, but they did not swab my rectum. I will try to get a rectum swab tomorrow at a clinic. I’m concerned abiut the negative results though. If a urine test is negative, does that mean it is not in my penis and is confined to my anus? If I do have a rectal std, is it possible that it was there long before, asymptomatic? Could I have passed it on through my penis? I assume not since the test was negative. Obviously it is most likely that I received something from my last partner, but I am concerned whether I should notify previous partners if I have had thus longer than I thought.

    • 1. Yes. Urine tests only tests for infections in the urethra. If the infection is in your anus, the urine tests will not pick it up.
      2. Yes. Many times, rectal STD infections are asymptomatic.
      3. No. If the infection is in your anus, you cannot pass it via your penis. Infection is site to site.

  14. I’ve been having anal discomfort. I in the past went to my Dr. And he would swab and check for STD’s. I just recently got new insurance and had to find a new Dr. I went to this new Dr. With these symptoms and he said he would take a urine sample to see if I have a STD in my anus.??? I told him I never heard of that for the rectum and he assured me the urine test would work. Is that true???

    • I am in no position to comment on whether what your doctor said was true or not. All I can say is, anal STD is best detected with an anal swab especially if you have anal symptoms.

  15. Dr. Tan I have informative story more than question. I have been taking PrEP, plus have HPV vaccine. This made me stupidly bold in having unprotected sex on vacation. My thinking- even if I get gonorrhea/syphillis/chlamydia – those are treatable if caught and I test for those frequently.
    So, I bottomed raw for a stranger, and since I’d sought someone of considerable penis size, he caused tearing deep inside my rectum. Unfortunately he also had gonorrhea which infected me and caused an anal abcess. Since it was deep inside, it was only detected via a CT scan at hospital; a doctor suspected abcess but couldn’t find it visually. I was in excruciating pain – could barely walk/sit. It was less than two weeks after the sexual encounter. I needed surgery to drain abcess / pus; the location required cutting deep into my tissue to reach. It was under local anesthesia as I had so much pain I didn’t want to wait a day longer. General anesthesia would be better as it’s easier to address – though that carries different risks. Now I am taking oral medication and scheduled for a shot this morning to address the gonorrhea (we discovered the STI yesterday post abcess treatment).
    Conclusion – gonorrhea is not the harmless STI we think as there can be complications as I learned.

    I was with a guy 4.5 weeks ago it was my first time and he must have put something in my drink bc I woke up to being completely taking all of him and h said he was 6-8inxhes in girth. 3 weeks ltr I had some pains and itches in my groin, buttocks, thighs, esp inside my butt. I have no warts or scabby skin or pimples or spots of any kind. The only thing is I have hemorroids and feels like I’m always pushing to go the potty, stomache pains infrequently, it’s been two weeks and all the inflammation has subsided but I still have one external hemorroids that’s healing but when I ejaculate it feels like someone has kicked me in the balls. I don’t want to go to the dr. This is way to embarrassing all I know is that my pretty little butthole is now red, raw, inflammed,irritated, hemorroidal, and just plan angry. What could it be. The main thing is the on my anus toward my back is a cartilage type white almost scar looking thing that hasn’t healed or gotten better. What could this be. It looks like the one of the valves to my heart has broken off and lodged in my sphincter. HELP!!

      That is very graphic but well written. First of all, I’m sorry to hear of what happened to you. I know you don’t want to go to the doctor but I am afraid you really do not have much of a choice. There could be multiple things wrong with you some of which potentially dangerous. What if he inserted more than his penis in you? What if the injury in your anus and rectum is much more serious than you think? What if he gave you Chlamydia, Gonorrhea, Herpes or God forbid HIV? I’m not trying to scare you but I am concerned for your health and the best advice I can give you right now is to see a doctor. There is really nothing much else we can do for you over the internet.

    • Yes. Fingering and rimming can transmit gonorrhea. Although I would say protected anal sex is more likely to be the cause.

    • Your symptoms sound like a STD. Please see a doctor to get yourself assessed and tested.

    I have discharge from my penis. I am positive it is gonorrhoea. I had unprotected anal sex. I am now travelling for work. I am leaving tomorrow. The Dutch doctor (I am at home at my family where I first discovered the symptoms. I will now be one week in the Ukraine, then two months in Slovakia. Will I leave it too late when I only treat it in Slovakia? They will do test, I presume and only then I will get antibiotics. This will mean I will only treat it in two weeks. It hurst when I urinate. Will this stop? Will I likely be left with problems?

      It certainly sounds like Gonorrhoea. Don’t worry it is not life threatening but you should seek treatment at your earliest convenience to cure it.

    I’m a bit confused as to what the discharge symptoms are for anal gonorrhea. When you say discharge, what color and texture is it supposed to be like? Will I be able to see it on a clean tissue paper or when I go to the bathroom?

    • It varies. I could be whitish to yellowish discharge. Consistency could be thick sticky to thin watery. It is most likely visible on a tissue paper.

    My partner and I are both clean. I had anal sex a week ago and my partner ejaculated In my anus after ward I didn’t have the usual semen discharge that I usually do and I’m positive he ejaculated in me and I’ve felt kinda constipated. Now here I am a week later and I’m barely seeing traces of his semen in my feses is this normal???

    • You are worried unnecessarily. And I think you got your priorities mixed up.
      You should be more concerned about using protection during anal intercourse, rather than whether the semen flows out or not after ejaculation.
      And even if the semen gets retained in the anal cavity, it does not cause any harm apart from risk of transmitting STD. If you noticed, semen becomes colourless after a few minutes in the room air because the protein gets broken down. I’m not surprised if you do not see traces of semen flowing out later on.

    I have a query: can we acquire a rectal STD via the use of bidets (handheld water shower) in public bathrooms? If I’ve used a bidet in a public bathroom to clean my rectum by spraying water into it, is it likely that I may have acquired an STD? Should I get tested? Or does the constant spray of water prevent bacterial organisms from staying on the handheld bidet?

    Btw, I’ve recently had itching on my buttocks near my anus, with a slightly raised rash.


    • It is rare but possible. If you have symptoms or are worried, consult a doctor and get tests done. You are welcomed to visit any of our clinics for consultation.

  26. I am a straight man never had sex in the butt and just spent 4 years in jail I have a burning itchy n white discharge coming out my anus. What can be going on and can it affect my penis?

      Bad news man – you can get STDs in the anus even if you did not have anal sex. Of course there are other possible causes of itching and white discharge but honestly the first thing we think of is STD. Can it affect your penis? Well, assuming what you have is an STD then you caught it sexually and it could well be in your penis too. Anyway, at this stage it is really premature to be jumping to conclusions. You should get the discharge checked out and find out what is causing it.

      • I had gonorrhea in my penis from a girl and i got the shot and took doxicycline for a week im good now in my penis but could it have spread to my butt even though im clean and was taking the pills and the shot?

        • No, it would not spread to your rectum; and if you had rectal gonorrhea, it would have been treated all the same.
