HIV testing may seem very complex.

This is greatly due to the testing ‘Window Period‘. That irksome time from when a person is infected to when tests can actually detect a HIV infection.

This has caused a lot of doubt and worry with regards to the accuracy of tests done.

You might even be wondering right now how accurate your tests are.

There is a lot of confusing information out there. Some people claiming a window period of 2 weeks some people even saying 10 years!

This is because HIV testing technology has developed significantly since HIV was discovered in the 1980’s.

So a lot of information out there is out-dated.

Also, not the same standard of tests are available everywhere. This has caused different governments and different health authorities to develop different testing guidelines. So the reason why the Window Period is different in Massachusetts is different from Singapore is not because humans in the US react differently to HIV compared to humans in Singapore. It is because of local guidelines.

So what exactly if the HIV Window Period?

It depends a lot on WHICH TEST was done.

The following graph was prepared based on the latest information on Testing for the Diagnosis of HIV Infection.

Some points to note:

1. If you are within the first 10 days of possible HIV infection (aka the Eclipse Period), don’t bother doing any tests. There are NO tests that are accurate.

NB: If you are within the first 3 days of possible HIV infection please see us about PEP.

2. At day 10, the HIV-1 RNA PCR can detect HIV Viruses in the blood.

Remember that the RNA PCR test has a high chance of giving a false positive result.

If the RNA PCR test gives a report of less than 5000 viral copies per ml, it is likely a false positive.

Also, the viral load may drop to an undetectable level upon full sero-conversion.

This is because the body’s immune system may have suppressed the infection to a point where there is not enough HIV virus in the blood for the RNA PCR test to detect.

3. Between day 14 to day 20, the HIV P24 Antigen will start appearing in the blood. This can be detected with 4th Generation tests (aka Combo tests).

This is why, although the official recommended Window Period for the Combo Test is 28 days, we are very confident when a patient tests negative at 2 weeks.

Remember that the P24 Antigen level will peak at about day 28 and becomes undetectable at about 6 weeks from infection.

This does NOT mean the 4th Generation (Combo) test is inaccurate after 6 weeks because the Combo test also detects IgM and IgG Antibodies (See below).

4. Between day 20 and day 23, the IgM Antibody starts to become detectable in the blood.

This is only detectable by 3rd and 4th Generation HIV tests.

This is the other reason why even though the 4th Generation (aka Combo) test is supposed to be conclusive at 28 days, at 3 weeks when a patient is negative, we tell him/her it is as good as conclusive.

5. Between day 28 and 48, the IgG Antibody starts to become detectable in the blood.

This is detectable by 1st, 2nd, 3rd and 4th Generation HIV tests.

This will remain detectable in the blood for as long as a person lives. Therefore, doing a Combo test not only detects HIV infections that occurred recently, it also detects infections from years ago.

More things to note:

The recommended 1st line screening for HIV infection is the Combo test NOT the antibody test.

So when you go for a HIV screening test, you should be asking for a Combo test. Take note that in Singapore the Combo test costs more than an antibody test.

The essential thing to remember about HIV testing is to get the RIGHT test done at the RIGHT time.

      Hi,

      Generally 4th Generation HIV test if done after 28 days post-exposure they are conclusive. If you are still concerned, you may want to repeat after 3 months post-exposure.

      Hi,

      As with most things in life, there is never 100% certainty. Given what you have said, I would say that it is very likely you were not infected with HIV especially with multiple testing up to 16 months post-exposure.

      • Hi doctor. I had protected sex on 17th Jan 2018.
        I also had (what appears to be) ARS symptoms on day 21 after sex exposure, with high fever (up to 39.5degrees celcius), sore throat, lots and lots of mouth ulcers like about 12, and a slightly swollen lymph node on my neck. I didn’t have cough, runny nose or blocked nose etc, which I know was typical of ARS because ARS usually doesn’t have cough/runny nose(correct me if I’m wrong).
        I got tested by AFA on my 17th day after exposure using RAPID TEST 4th Gen Combo test (NEGATIVE)
        I also went to your clinic at Somerset to test on my 29th day after exposure using RAPID TEST 4th Gen (AG/AB) Test (NEGATIVE)

        I have some questions:
        1. Are my results conclusive? I read online that it may take up to 10days(from date of seroconversion) for 4th gen HIV test to be positive, not sure whether true or not.
        2. 4th gen duo rapid test vs 4th gen lab duo test, are they equally conclusive? should I be tested using lab test since I only did rapid test?
        3. Do I have to worry about my status? I had a big shock when I fell ill 21 days after exposure.
        4. I also went for Gonorrhoea/Chlamydia PCR test & Syphilis blood test on 29th day after exposure. Because my urine & blood sample were drawn on CNY eve(which was thurs) they were only delivered on the next available working day(which was next Monday). following this long break before samples were sent to lab, I also had to wait about additional 2 weeks before my results were out(NEGATIVE results). Will the long wait cause the results to be inaccurate in any way?

        Ultimately do I have anything to worry? Please advise. Thank you!

          Hi,

          Generally, if you done your HIV testing with 4th generation rapid test 28 days after exposure, it is conclusive. Your symptoms may not be due to ARS but if you still are having symptoms such as ulcers in the moth, you should consult a doctor for further evalaution as you could have other STDs such as herpes. The Gonorrhoea/ chlamydia PCR you did is conclusive despite you doing the test on CNY eve, it has no effect on the results.

          • Hi doctor, thanks for the reply.
            Regarding Herpes, my ulcers inside the mouth cleared up within about 7 days. I did not have blisters on my lips or outside my mouth, it was just purely oral ulcers inside the mouth. Also, I read that the incubation period for HSV-1 is from 2-12days, but I fell sick at 21 days after exposure. Thus, is it still possible I could have gotten herpes?
            Would the ‘Comprehensive Male STD Test’ provided at your clinic be conclusive result for all STDs when I go for testing at 90days after exposure? Or should I wait a little longer?

            It is still possible for you to have herpes within a month period. There are 2 types of herpes. Certainly, you can do testing for various STDs including herpes after 90 days if you are concerned.

      Given that you have repeated testing 16 months after the exposure, it is almost certain that HIV was not transmitted. However, if you are still concern please visit a doctor.

      Hi,

      In general, if the 4th generation HIV test done was negative after 1 month post-exposure, it is conclusive. I do not think that you need further testing unless it is to relief your anxiety.

    I met a ktv girl and brought her to a hotel on 18th March. I ejaculated during the handjob before any vaginal sex and thereafter, I do not have any penetrative sex. However as we are lying down naked, I did experience my foreskin being pulled back (I am not circumcised) during handjob. I am not sure if my penis tip has came in contact with her vagina or it is due to hand movement. Her vagina wasn’t wet on the outside when I touched it.

    I would like to check if I need to wait 28 days to go for 4th gen check? For advice please. Thank you

  14. Hi Doc, thank you for your work on this website
    I have two concerns :

    -I had some unprotected vaginal insertive intercourse with a girl 29 days ago
    -I also posibly had some receptive anal intercourse 15 days ago. Circumstances are weird because I seriously blacked-out in some party, but I remember being with a guy and I felt some anal discomfort for the following days. I have never been anally penetrated before, so I think I would have remembered/known it for sure, right ? While it is much more probable that i’m being paranoid on this one, I prefer to ease my mind with some tests.

    To sum this up, my two risks are at 29 and 15 days ago, I tested today. How confident can I be if the test turns up to be negative ? (I had a 4th generation duo test)

    I understand from the above article that the 29days ago risk could be discarded, but how likely is a 4th generation negative test at 15 days to turn positive at 28 days, from your experience ?

      As you mentioned, the HIV 4th generation test has a window peroid of 28 days, thus if you test within the 28 days window period, you will have to retest after 28 days from the last exposure.       Hi,

      There is no way to predict if someone will have delayed seroconversion. However as I mentioned all the tests you have taken thus far for HIV has been negative thus I would say that they are conclusive.

      Hi,

      For vast majority of the HIV infection, using the 4th generation HIV test 28 days post-exposure is conclusive. It is not possible to say with certainty the risk of HIV transmission after a single exposure as many factors such as whether the partner is on HIV treatment and also how high the viral load is. Given the fact that you did the HIV RNA quantitative test, 4th Generation HIV tests 15 months post-exposure and the results are still negative. I would say that it is conclusive.

  19. hi Doctor I am Male from uk who performed unprotected oral sex on a female and she performed unprotected oral on me few weeks later I had ARS like symptoms and ever since had all kinds of symptoms, I tested in a uk GuM clinic at 6 weeks 8 weeks 9 weeks 11 weeks and 14 weeks which I was told they were all 4th generation tests and they all came back negative, I have tested for all other stds an all came back negative but I still have all symptoms. My question is would I have to wait 6 months for a fully conclusive tests or are my recent tests definitely conclusive?

      Generally, 4th Generation HIV testing was done 4 weeks post-exposure, it is conclusive.

    I have tested STD gonorrhoea negative HSV1 positive and taken medecines Zovirox for 1o days and antibiotics for 15 days during the 1st month
    Do my results conclusive I am in west Africa I read HIV2 had late seroconversion. I am very worried about all my symptoms I have never had illness before I was healthy I am 45 yrs. old did I infected and not detected by all these tests do I do go for western blot test or any other test Pls help me Thank you

      Hi,

      In general, oral sex carries a very low risk of HIV transmission. Together with the fact that you had been tested multiple times over > 90 days post exposure for HIV 1 and HIV 2 which were negative, the results are conclusive.

    I had unprotected sex with an NRI woman on January 23, 2018 early morning (or midnight) about 5 times and 1 time in the morning.

    Now, yesterday, on February 21, and today on February 22, 2018, I went for a 4th generation antibody and 4th generation Duo Combo test respectively. They are 29 and 30 days respectively. Both are negative and non-reactive.

    The woman in question since returning to her country got heavy DVT/PE and keeps coughing since. It has been two weeks. She returned to our country again and was sick like a dog. She was on blood thinners.

    Now, she reports a sharp and unbearable headache with ENT complications during take-off and landing. She feels dizzy, fatigued and unable to walk at times. Coupled with that she had dysuria during menstruation and many other symptoms.

    The female AIDS survivor (Jennifer Vaughan) who became famous on YouTube also said that she had severe stabbing headache during take off and landing. So, I would like to know if these could be late stage HIV symptoms?

    And what about my results doctor? Were the right tests taken at the right time? Are my results conclusive? Please reply me as early as possible. Thank you doctors!

    • HIV symptoms are non-specific, and can be due to multiple other conditions. I cannot comment on HIV probability just through symptoms. I would consider your results conclusive though, so I would not worry if I were you.

        I found myself another reason to worry though. The 4th generation test was a home blood sample collection from a lab.

        The guy took out a syringe pack in a cover and tore it open, which I saw but since I was a few feet away, and didn’t personally touch it, I feared I would contract it through the syringe. The lab technician reassured me enough that it was new.

        While filling it in the tube, he also sort of exposed the blood to air. So…

        1) Should I go on PEP again due to fear of the syringe? I was on PEP till the 30th day, beginning at 67.5 hours.
        2) So, even with PEP, 30 days is conclusive?
        3) Does exposing the blood sample to air dilute the result? Or change it in any way?

        Sorry for wasting your time. I have a lot of OCD as well. Can you please reply Dr Jon? I promise this is my last.

        • No, I would not consider PEP for the testing itself, otherwise you will be paranoid forever. Testing at 30 days even with PEP is essentially conclusive, although guidelines will still recommend one more at 3 months for precaution. Exposing the blood to air in that way will have no impact on the result. Move on with your life.

            Hi Doctor,

            I’d been happy to have no symptoms but I now suddenly ruined my health and started getting symptoms.

            In week 8, I had blood in stools just once, I strained a lot during the bowel movement and developed a pea sized swollen lymph node on the left side of groin. I also felt a similar one on the right.

            Coming back from home to where I stay (a new place), I went out a night for dinner after 10.30 PM. That day it had rained and there was some fog/mist around. I felt a bit cold.

            The evening before, I had watermelon juice. The night I went out for dinner, I had a cup of yoghurt. Before that, I smoked a light cigarette that I knew to cause cold but since it was given by a friend I smoked it.

            The new place has a lot of mosquitoes. One of them went straight into my mouth while sleeping and I coughed it out. The next evening, I again went out and sneezed almost every few seconds. It was cold outside. I couldn’t have dinner comfortably.

            When I came back home, I had severe sneezing, sore throat, runny nose, pain, irritation and petechiae on soft palate, occasional pain and discomfort in tonsils, irritation in tonsils, if I just swallow without any food, occasional cough to clear throat, slight to a lot of mucus, one to later more than a few white dots on tonsils, etc.

            I had a swollen tonsil on my left since before a couple more days.

            As time goes by and out of loneliness, I get convinced my results are going to be adverse.

            I think I had one swollen lymph node behind my left ear, which I am not sure if it is a skin/muscle bulge due to my straining bowel movements or a swollen lymph node. The pea sized lymph node on thighs went down in size today (in two days).

            I even picked the needle from the box after which the phlebotomist kept the box inside his bag to show that he used the one I picked. He left the needle used on me on the sofa.

            Since then I got scared that the random needle I picked could have been forgetfully kept back in the box after use and I picked it.

            1) Are these symptoms due to my first exposure showing up after 54 days since I was on PEP (24 days and 3 to 4 weeks after PEP)?

            2) Are these symptoms due to my second exposure of the fear over a used syringe? I got duo/combo tests 41 days (11 days after blood draw & 12 days after PEP) and 49 days (19 days after blood draw and 20 days after PEP), both negative.

            3) Were my 49 day results conclusive for the original exposure, though on PEP, and 19 days after the blood draw?

            4) Was the blood draw any risk at all?

            Sorry doctor, fear, anxiety, loneliness and other emotions trigger all these phobia and paranoia 🙁

    • Day 17 testing is a good indication but not conclusive. Please repeat one at 28 days or more.

    • The test at 34 days is conclusive, no HIV. The syphilis testing is a good indication but only conclusive after 3 months.

    I had done protected s*x with Unkwn HIV status lady. After a week I experienced flu symptoms like 24 hours headache , mouth ulcer , chill , muscle ache . I got worried and went for HIV 1 & 2 (Ag & Ab) test after 68 days it’s came as negative. But she still I have same headache (24 hours) , muscle ache and mouth ulcer. No fever at all.

    I had unprotected sex with sex worker on 17/10/17 and after 7 days I had hiv 1&2 test which is non reactive, than I had 4th generation test hiv1/2 & p24 combi (cmia) after 26 days & 55 days both are also nonreactive (index value is 0.09 & 0.10)
    1 – I just want to confirm if the p24 combo test i has done after 26 days is non reactive then it can be possibility that I have hiv.
    2 – Do I believe that p24 combi test after 26 days & 55 days is a conclusion that I don’t have hiv and I should be relax and don’t required more test.

    Please clear all my query as I am too much worried about the test conclusion result that are that accurate after 55 days.

    • Your latest test at 55 days is conclusive. No HIV.

        There will be no further problem to my wife after making sexual relationship with her, as is that fully clear that I don’t have hiv and no need for further test.

  41. I had a female massage parlor girl insert her finger in my rectum massing my prostate. She did not use any protection. She may have hiv and bled in me. Since then have felt ill and now developed black tarry stool which we presume is ulcer. I tested with combo duo test at 20.5 days after exposure and was negative. How reliable is that result even with symptoms.

    • Your risk from this exposure is essentially zero. However, for conclusive HIV testing, I would recommend a repeat at 28 days or more post exposure. If you still have symptoms of concern, please visit us for a proper consult and assessment.

    I am male from the US and my exposure was low risk I believe. Urethra came in contact with vaginal fluid through hand job she have after touching her vaginal fluids, so outside the body. Since then I have had 4th Gen Abbot Architect tests after 8, 21, 36, and 53 days of exposure. All 4 tests negative. I also had HIV PCR RNA on 36 day which was undetectable. Are these tests conclusive? I have had electric sensations and pain in fingertips/legs/toes for a month now and they won’t go away which adding to anxiety. Please help!

    • Your tests are conclusive and your initial risk is zero. If you have symptoms of concern, please have them assessed by a doctor.

        • Your tests are conclusive – no HIV. If you are still having anxiety, you should consider speaking to a counsellor/psychiatrist.
