I have been getting a lot of question with regards to the Duo test or Combo test as it is also known as.

Let’s first get some basics right:

Duo Test = Combo Test = P24 Ag/Ab test

All these terms refer to the same thing. Basically it is a test that simultaneously tests for the HIV P24 Antigen and the HIV antibody.

The HIV P24 antigen is a small piece of protein that is found on the capsule of the HIV virus. When a person is infected with HIV, these bits of protein can be found floating in the blood. The HIV P24 test is a test that detects these bits of protein. This test was first developed as a HIV screening test but rapidly ran out of favor due to the development of more advanced NAAT tests. This test alone is only accurate for between 3 and 6 weeks post exposure. So it is a test with very limited applications unless combined with the HIV antibody test. The presence of P24 Antigens in the blood indicates a recent HIV infection.

We all already know that the HIV antibody test becomes quite reliable at 4 weeks and as good as conclusive at 6 weeks. Although most health authorities still recommend a 3 months window period. So putting these 2 tests together gives you a very powerful tool to diagnose HIV infection in as little as 3 weeks. Personally I prefer to conduct the Combo test at 4 weeks because I believe that is when the level of P24 antigen peaks in the blood giving us maximum accuracy.

In our clinic, we use the SD Bioline Rapid HIV Combo Test 2.0. It is a rapid test that takes only 20 minutes for us to get results. We are also approved to conduct this test as part of the Anonymous HIV testing Program.

  1. Hello. Ive received answers from alot of blogs and paid medical forums saying that i have no risk. Its regarding hiv transmission on my shin (im afraid that vaginal fluid may have entered the skin) eventhough the skin upon inspection is intact. im worried about microwounds.

  3. Dear Dr…Is 4th gen duo test conclusive at 28 days?….Some specialists saying it’s conclusive at 6 weeks…Some saying it’s at 3 months….It’s confusing….Please tell me why different opinions…..Which one we should follow.

    • Our opinion is that it is conclusive at 28 days. At the end of the day we are also just another expert, so the best place to go is your own medical doctor.

  4. Sorry to add on, but I had a ache and sore feeling on my gut are and the area between my butt and the testicles on the 1st week. The std clinic doctor told me to find another doctor for that. Could it be due to me trying to poop very hard for a few times while I was overseas? I visited the doctor he gave me antibiotics and it was gone after a few days. But at the 6th week it come back again. Only for a day though

  5. Had protected oral sex with sex worker(mouth to penis). Went to get tested for comprehensive urine check after 1 week,results were negative. Then had a Duo combo rapid test at 29 days after exposure. Result tested negative but there were some blood stains on it,but don’t think I saw the lines that state I am HIV positive,the nurse there told me that I tested negative. Do you reckon I need another test? Or am I cleared? Thanks

  7. Hi Dr, i tested negative at 29 days post exposure with 4th generation ag/ab hiv test. My question is how you are so sure that it is conclusive after 28 days? because UK guidelines and CDC guidelines all stated that the result at 4 weeks only 95% accurate and we need to retest again at 6 weeks and 3 months. They said some people need more time to develop antigen n antibody

  8. My duo test on 24 days, 30days. 38ays conclusive for hiv. And other std check at six week conclusive at 40days conclusive?

    • The HIV tests are conclusive. The other STD check will depend on what was checked. But I would say the majority of STDs can be picked up by 40 days post-exposure.

  9. Hi Dr, i have done 4th generation ag/ab combo test with gribbles company in malaysia at 21 days(3 weeks) post exposure and it turn out negative. Can i rely on the result and expect it wouldn’t change in further test?

  10. Hi dr Tan, Went to your anonymous clinic last week and did the alere hiv combo test. Did the test at day 34 after exposure. Would like you to confirm that the negative test is a confirmation that I have no hiv. Thank you.

  11. Hi DR.. I had sex with a sex worker 2 weeks before.. I did CMIA test on 15th day and the result is non-reactive. My question is how many percentage is the result is accurate? Thank you

    • If your test was 4th generation and included antigen and antibody, it is a very good indication, but still not 100% conclusive. Please repeat a 4th gen test at 28 days or later.

  12. dr, just follow up, i just done a combo test after my exposed 21days and currently i have hives on my skin (around 1 weeks). the 21days combo test is negative and can i say that the rash that i hving is not related to ars?

  13. Hi dr, 2 weeks ago i was exposed to csw without any sex and oral. I was hj myself with condom. Currently i was hving a skin rash something like hives and will go for cpmbo test at your place.
    1. Hives is the ars?
    2. I was hj myself and the girl may touch my body is tht risk?
    3. Combo test at 21days is good indicate for my case as without any penetration?
    4. The ski rash like bump, become red then will slowly disappear and itch at night.is this the skin rash ars? It look like a large bump.

    • From the exposure you described, the risk of HIV transmission is zero. Hives can be caused by many things. If you are still concerned, please come down to our Robertson Walk clinic for a consult and evaluation of your rashes.

    I had unprotected sex with my new girl friend on Apr 12 this year. She’s VDRL and Syphilis test March 7 was non reactive. I don’t know she’s current Hiv status but according to the employment last year June was non reactive. Any way I take Hiv p24 antigen test at 28days now waiting for the result. This antigen test result is whether conclusive or not. And then I already have plan to come your clinic for HIV and STD test screening. Please help me reply thanks.

    • The combo antigen+antibody test would be conclusive at 28 days. But you are welcome to come by our clinic for further consult and testing.
