I have been getting a lot of question with regards to the Duo test or Combo test as it is also known as.

Let’s first get some basics right:

Duo Test = Combo Test = P24 Ag/Ab test

All these terms refer to the same thing. Basically it is a test that simultaneously tests for the HIV P24 Antigen and the HIV antibody.

The HIV P24 antigen is a small piece of protein that is found on the capsule of the HIV virus. When a person is infected with HIV, these bits of protein can be found floating in the blood. The HIV P24 test is a test that detects these bits of protein. This test was first developed as a HIV screening test but rapidly ran out of favor due to the development of more advanced NAAT tests. This test alone is only accurate for between 3 and 6 weeks post exposure. So it is a test with very limited applications unless combined with the HIV antibody test. The presence of P24 Antigens in the blood indicates a recent HIV infection.

We all already know that the HIV antibody test becomes quite reliable at 4 weeks and as good as conclusive at 6 weeks. Although most health authorities still recommend a 3 months window period. So putting these 2 tests together gives you a very powerful tool to diagnose HIV infection in as little as 3 weeks. Personally I prefer to conduct the Combo test at 4 weeks because I believe that is when the level of P24 antigen peaks in the blood giving us maximum accuracy.

In our clinic, we use the SD Bioline Rapid HIV Combo Test 2.0. It is a rapid test that takes only 20 minutes for us to get results. We are also approved to conduct this test as part of the Anonymous HIV testing Program.

Many women are confused when it comes to vaginal discharge.

However, often this isn’t a topic you would openly discuss with your friends or even your doctor.

You may have questions such as “ Is it normal discharge? Why does my discharge always vary in amount and colour? Should I see a doctor? When should I be worried?”. These are perfectly common and normal questions to ask as vaginal discharge varies from individual and they can be quite confusing.

Usually there is really nothing to worry about unless your vaginal discharge is associated with itching, pain or bad smell. Having vaginal discharge is normal in all women. But of course, it will be helpful to know what normal discharge is or looks like.

Vaginal Environment

Before proceeding any farther, let us realize that a normal healthy vagina is never free from a type of bacteria known as ‘lactobacillus’. When balanced in amount, it is a ‘friend’ and ‘protector’ of the vagina. Its role is to protect the vagina from infection by releasing certain proteins and maintaining a vaginal pH of 4.5. Once the bacterial count decreases, vaginal pH changes and various infections of the vagina can occur. Green, yellow, gray and white, thick, cottage-cheese like discharge, usually with odor, itching and pain are signs of infection.

Normal and ovulation discharge

Clear, whitish, thin or thick odorless discharge are usually normal and shoudn’t be alarming. It can happen at any time during your menstrual cycle. This discharges means your reproductive organs are ‘cleansing’ the vagina and cervix, getting rid of dead cells and bacteria which is being transported by these discharges.

Somewhere around the 14th day of your cycle (i.e when an egg is released from your ovary -termed ovulation), you may notice clear colourless and elastic discharge resembling egg white. It is usually much more in amount than your usual vaginal discharge. This is a good sign for those who are trying to concieve as this when you are most fertile.

Sometimes, white and more watery discharge can be found after heavy physical activities, when you are sexually excited or sometimes when you feel extremely tired.

Common causes are Infections, Hormonal Imbalance, Vaginal Environments Changes and maybe it’s just Normal Discharge!


Menses discharge

Besides having bright red blood discharge during menses, you can also have dark brown discharge which usually occurs towards the end of your menstruation or 1-2 days after menstruation.  This is the leftover menstrual blood in the vagina which was not completely cleared. It is brown as dead blood cells turn brown. Sometimes, this dark brown discharge can also be seen during your mid cycle when ovulation occurs and is normal.

Bacterial vaginosis (vaginal infection caused by bacteria) is one of the most common causes of gray, foul fishy-smelling vaginal discharge. Redness of the vulva, itching and burning sensation during intercourse may accompany. Green, thick fishy-smelling discharges are commonly caused by gardnerella infections.

Gonorrheal infections are manifested by cloudy yellow discharge often accompanied by pelvic pain and frequent urination.

Trichomoniasis (a parasitic infection) gives yellowish frothy discharge with bad odor. This is usually transmitted through unprotected sex. These infections are commonly treated with over-the-counter antibiotics, most effective one being metronidazole in forms of tablets, vaginal gel/creams and suppositories.

White, cottage-cheese-like discharge often accompanied by itching is due to yeast infection known as candidiasis.

Other type of infections that you could get which could cause abnormal vaginal discharge includes chlamydia, ureaplasma and mycoplasma.

If you are pregnant, consult a doctor before taking any medications. Pay attention to your personal hygiene. Changing your panty liners frequently, keeping your feminine area cool and dry by avoiding tight clothing and linen panties may help prevent from such infections.

We must not overlook some other simple facts that may lead to changes in normal conditions of vagina. Decreased bacterial count and an increased vaginal pH more than 4.5 are the main culprits causing infection. You may not notice that using fragrant soaps, vaginal douching and frequent bubbly baths may be some of the causes. For those who are sexually active, sticking to one partner is the best prevention, as STDs are often associated with such infections. Pregnancy and menopause may sometimes lead to imbalanced vaginal conditions due to hormonal changes. It is then best to maintain a good personal hygiene.

Although less frequently red discharge or spotting is a more serious case. When this is noted, question yourself: Have you had any unprotected intercourse recently? If yes, this may be a sign of pregnancy and you might want to do a pregnancy test. Spotting during early pregnancy may mean miscarriages or threatened abortion. Some other reasons are vaginal tear and lacerations during rough intercourse, and, if you are more than 40, it may be due to cancers. This is the time when you should be worried and tell your doctor.

    • Yes, you can get tested now and it would be conclusive for both.

  2. Hi dr. I just go through very hard in this 1 months 3 weeks after i had protected sex with a girl. I was worried about all the test i done but my dr say i am ok clear but i am still worried so i really need some second opinion.

    2. I had hiv 1 and ag/ab on 27days post expose with the lady as well. Negative
    3. 28days hiv 1 and 2 ag/ab negative

    4. As i read the website you post hep c prolong window period. i had myself pcr rna quantitative at 34days. Not detectable.

      If you have concern about whether if you are HCV positive, you should be retested 3 months post-exposure as it may take up to 3 months. As to whether your HIV test is conclusive, that will depend on your HCV status. If you are not inefcetd with HCV then your HIV results are conclusive. If you are indeed HCV infected, then the seroconversion may be delayed and thus it is recommended that you should repeat the HIV test 6 months post exposure.

      • Dr julian cn my hcv pcr rna quantitative conclusove since you guy mention the hcv pcr rna is conclusive if done afrer 2 to 3 weeks. So how about 4 weeks onward( 34days)
        ? Is the hcv you guys mention in blog is regarding qualitative or quantitative?

    • Yes, it is considered conclusive from 4 weeks onwards.

        My suggestion is for you to do the HIV confirmatory testing (Western blot) and/ or HIV RNA PCR testing to resolve the discrepant results.

  4. I have an exposure on 8th Mac 2018.
    And i get tested on 2th April 2018 which is 25 days after exposure.
    i tested all STD test result shown negative.
    am i free from syphlis etc.?

      The suggestion is to repeat the syphilis test 3 months after exposure if concerned. Please visit a doctor earlier if you notice any blister or sores on the genitalia area.

  5. hi dr just wanna know my test accuracy and anything should i need to retest for my exposure. i had protected sex with a girl.
    1. we tested hiv 1 and 2 ag/ab at the day of exposure= nonreactive;
    2. i tested hiv 1 rna pcr real time at 10days from the exposure;
    3. we tested again with hiv 1 and 2 ag/ab at 27days post exposure: nonreactive,
    4. i tested myself with hiv 1 and 2 ag/ab at 28days from the exposure nonreactive.

      Based on your test results, they are conclusive. If you have concerns, you may want to consider retesting after 3 months post exposure.

      Positive … I swear she positive i do for her raped test after sex …and condom not broke …i testing condom in water after sex its good …now im on pep ..I can not eat and I can not do anything … I’m very scared..my question?
      1-When can I take a test ..Knowing that im on pep
      2-which test I must do…antibody or pcr or p24 antibody 4th…remamber im on pep now

        The risk if HIV transmission via oral sex is very low but in view of the HIV status of the partner, you can consider doing a preliminary 4th Generation HIV test at 2 weeks. Subsequently, you can repeat at 4 weeks and 12 weeks even if you are on PEP.

      PEP does not affect the results of the HIV tests. You may consider doing 4th Generation HIV test 2 weeks after exposure. However, you will still need to repeat it after 4 weeks post-exposure and it is recommended in your situation to do a final test 3 months after exposure.     Received a negative ‘not detected’ 4th Generation AG/AB at 5 weeks post potential exposure. Could you confirm if any topical steroid creams may affect this? I’m worried they may have reduced my immune functions by accessing blood stream. Am I right in saying that P24 would still be detectable at this time had antibodies not become detectable yet? Only used for 3/4 days and stopped 4 days before the rest.

      Steroid creams do not affect the results of the HIV tests. I would consider your results as reassuring.

    1. Is the peps will affect the accuracy of pcr rna result if i only taken one day?

    3.is the few hours different will make a diff for the test accuracy? 10days (the real time is 9days20hours)

    Hi doctor, i got test at dtap kuala lumpur last feb (almost 7month post exposure) using sdbioline 4th gen. It was negative. The test just in 5-10 minute. Is the test conclusive?.
    Befpre that, I been test negative many time before using rapid and lab test. Can i move on.

      If you have been tested on multiple occasions with the 4th generation HIV tests and were all negative esp 7 months after exposure, it is conclusive. Move on.

  12. Dear Doctor

    I am really having a difficult time due to my anxious nature. I went to a dentist in Tokyo for root canal treatment. He gave an injection to numb the tooth and did some serious dental work. During second visit after 8 days, he did some filling but no injection. After two days i.e. 10 days from first visit (when he gave an injection), I got fever for one night, one sided sore throat and swollen lymph nodes. I went to dentist, he said it is not due to dental work, but may be due to some viral or bacterial infection. Gave some antibiotics, it cleared way in one day. After finishing antibiotic course for 3 days, sore throat came again but not that sever, I started antibiotic again, it went away.

    I was terrified due to symptoms as they came together and were suggestive of ARS. I took HIV Ag/Ab test on 18 days from dental work (10 days from onset of symptoms), it came back negative. It gave me relief for some time, i started worrying again.

    After 50 days from dental work, I took HIV 1 RNA (Taqman PCR method detection limit 20 copies/ml), HIV antigen/antibody, Hep B surface antigen, Hep C antibody, Syphillis : All Negative.

    DO i need some more testing like at 90 days. What else can i do to establish that I am 100 percent HIV negative?

    Hello dr tan. I had a small risk from blood of hiv+ person. It got contact to my wound (4 days old without scabbig). Tested at 45 days with eclia antibody test and 84 days with rapid prick combo test. Both result were negative. But i a have a whitish/dry skin lesion on my penis head that sometimes hurt and also swolenn lymph node at my neck that is sometimes painful. Could it be hiv related, and is my test conclusive? Should i need another testing?

      Generally, if you test with HIV rapid Combo test (4th Generation) after 28 days, it is conclusive. However if you are concern about the whitish/dry skin lesion on the penis and lymph node, please consult a doctor for further evaluation as it is difficult to determine what it could be without seeing you.     I am experiencing fever, sore throat and fatigue, which is some of the symptoms of ARS, 2 weeks after a high risk encounter.

    If I take a 4th gen test now, would the test be able to detect p24 antigen or antibodies?

    Or should I wait till 28 days before taking the test ?

    Please advise and I will plan my trip to your clinic accordingly.


    • If you are having ARS symptoms, then the test would be able to pick up p24 antigen and/or antibody already. So we can do a test now if you are keen. However, for conclusive results we would still advise 28 days.

  16. Doc, can i have an answer please?
    The 4th gen combo test 9s valid after 3 years- 10 years-20 years from exposure Or it has to be sometime close to the exposure?

    Regarding the 4th gen combo, the antigen will not drop to undetected levels until there is enough detected antibodies right? Incase of seroconversion delayed so antigen will be detected and there will be no gaps?

    • It is valid any time after 28 days. It does not have to be close to the exposure. Antigen will be detected if there is no antibody.

    i tested hiv antibody only tests by using ‘Alere’ 24 , 75 , 80 , and 94 days after exposure were non reactive.

    I tested hiv by using hiv ag/ab combo test @ 115 days after exposure was non reactive.
    So, do i need another test?
    Are they conclusive?

    Thank you very much for running this forum and greeting from Japan. My exposure happened three months back in India. I visited a massage parlor for naked body to body massage. I was not involved in anal, vaginal or oral sex. I did few activities mentioned below:
    1. I sucked the nipple of the lady briefly 2-3 times.
    2. I rubbed her vagina with my finger and after 2-3 minutes , i masturbated myself using the same hand.
    3. I cleaned my penis using a towel which may had some infected fluid from previous customer.

    As I was terrified, I did some testing:
    Alere HIV combo @16, 26 and 43 days, blood was taken from arm and result was given after 20 minutes: All negative

    At 57th day, Laboratory based antigen/antibody test (CLIA) blood was taken from arm and result was given next day: Negative

    At 69th day, Laboratory based antigen/antibody test in Govt testing centre in Tokyo, blood was taken from arm and result was given next week : Negative

    At 90th day, Laboratory based HIV 1, 2 with P24 antigen (CMIA) test, blood was taken from arm: Negative

    At 106 days, Laboratory based antigen/antibody test (CLEIA), blood was taken from arm and result was given after 2 days : Negative.

    At 17th week after exposure, i developed soar throat with redness at the roof of the mouth. As i already did lot of internet surfing, I am worried that I may be a case of delayed conversation.

    Do I need to continue testing upto six months or do i need some advanced testing like NAT testing to make sure that I am 100 percent HIV free. This episode has entirely ripped off my personal and professional life and I am approximately hitting the point where I can loose my job and wife both.     • Your tests are conclusive and excessive – no HIV. I would advise seeking help from a psychiatrist.

Based on my risk and testing done afterwards, Can I be 100 percent (not 99 or 97 percent) sure that I am HIV negative?     I had unprotected sex and I did the combo test in your clinic in malaysia the sd bioline ag/ab test. I did it on day 14,23 and 38 and they are negative shull I repeat again after 3 months or can I forget about it

  28. I recently had protected sex with an unknown women. The sorting part is I out on the condom after I had fingered her, with my hand that is full of timber virginal fluid.
    Also after sex I remove my condom and than I touch my penis.
    I had tested on day 14 with ALERE combo AG/AB test and it is negative. What does the result means? When is the earliest I could do a conclusive test? I am very worried as I for a pass few days I am having sweaty chest in the middle of the night.

    • At 14 days, the test is a good indication, but for conclusive results you would need to repeat one at 28 days or later.

    Looking for some advice. I had unprotected insertive vaginal sex with a sex worker in the UK. I’m a circumcised male and there was no visible blood or trauma.
    I spoke to her 4 weeks post exposure and asked her regarding her hiv status. She said she is tested monthly, her last test being 2 weeks ago (so 2 weeks post exposure) and that she was negative. Said she had certificate and documents from the hospital. Obviously I’ve not seen this and can’t say what she said is 100% the truth.
    I’ve had the following tests:
    14 days post exposure – Alere HIV Combo (antibody/antigen test) (20 min result) – Negative
    21 days post exposure – Alere HIV Combo (anti/antigen) (20 min result) – negative
    28 days post exposure – Alere HIV Combo (anti/antigen) (20 min result) – negative
    35 days post exposure – Alere HIV Combo (anti/antigen) (20 min result) – Negative
    38 days post exposure – HIV DUO Lab test (antibody/antigen) (next day result) – negative
    42 days post exposure – Alere HIV Combo (anti/antigen) (20 min result) – negative

    The doctor said to me that since I’ve not had any obvious HIV symptoms and that all the tests that I have had are negative I can forget about hiv.

    Just wondering if that’s correct advice or am I right to still be anxious?

    Do I need to retest again at 8 or 12 weeks?

    i made a test HIV ag/ab 1+2 and the result came 0.58
    i did the test before and the result was 0.12,0.14,0.10

    the result has been done after 29 days but my question why is that the number getting higher ?

    • There are other similar non-HIV antibodies that can be detected by the test, and a margin of error allowed for detection. As long as it does not cross the threshold for “reactive” you are fine and do not have to worry. If you have further concerns, please speak to the healthcare providers that administered your testing.

    • Our opinion is that it is conclusive at 28 days. At the end of the day we are also just another expert, so the best place to go is your own medical doctor.

  40. Sorry to add on, but I had a ache and sore feeling on my gut are and the area between my butt and the testicles on the 1st week. The std clinic doctor told me to find another doctor for that. Could it be due to me trying to poop very hard for a few times while I was overseas? I visited the doctor he gave me antibiotics and it was gone after a few days. But at the 6th week it come back again. Only for a day though

  41. Had protected oral sex with sex worker(mouth to penis). Went to get tested for comprehensive urine check after 1 week,results were negative. Then had a Duo combo rapid test at 29 days after exposure. Result tested negative but there were some blood stains on it,but don’t think I saw the lines that state I am HIV positive,the nurse there told me that I tested negative. My question is how you are so sure that it is conclusive after 28 days? because UK guidelines and CDC guidelines all stated that the result at 4 weeks only 95% accurate and we need to retest again at 6 weeks and 3 months. They said some people need more time to develop antigen n antibody

    • The HIV tests are conclusive. The other STD check will depend on what was checked. But I would say the majority of STDs can be picked up by 40 days post-exposure.

    • If your test was 4th generation and included antigen and antibody, it is a very good indication, but still not 100% conclusive. Please repeat a 4th gen test at 28 days or later.

    • From the exposure you described, the risk of HIV transmission is zero. Hives can be caused by many things. If you are still concerned, please come down to our Robertson Walk clinic for a consult and evaluation of your rashes.
